also the 'humans' are just scapegoats for organizational and institutional pressure to shorten judgement time to the minimum amount possible. you set someone up for failure, they will fail, and if you put all the blame on them, allowing organizations and leadership to continue as they are then you're guaranteeing that it'll happen again

Americans have a hard time thinking about problems and issues without making it all about individuals. no systems-level thinking to be found for miles (and I say this as a somewhat regularly frustrated American)
