The strike on the girl's school in Iran is an early example. That one happened when the AI didn't even have direct control of the weapons. All it took was people starting to offload their thinking to AI which we've seen is becoming increasingly prevalent.

With fully autonomous weapons being under development, the scenario becomes even bleaker.
