Often you pay to have that posting. For a smaller company they def know that posting is still up. Likely they are just getting poor applications (or need to improve their posting because the right people are not seeing it)

In the age of LLMs every one is "tailoring" their resume to every posting.

This depends a ton on if we're talking about a company's own website's job listings or if it's on some dedicated job listing site.
